Gastric cancer has become one of the important public health issues endangering people's health in China. Currently, endoscopic submucosal dissection (ESD) has been used as the curative procedure of early gastric cancer without lymph node metastasis. Endoscopic resection has several advantages, including less invasiveness, permitting en bloc, histologically completed resection, accurate pathological diagnosis, lower recurrent rate and rapid recovery. The perioperative period of ESD is the time of a patient's endoscopic operative procedure; more specifically, it includes ward admission, indication, contraindication, preoperative preparation, endoscopic operation, postoperative complications, recovery and the disposal of specimen. The aim of this guideline is to assist endoscopists in providing standardized operation to patients, as well as managing perioperative complications.

The fifth national consensus conference on the management of Helicobacter pylori (H.pylori) infection organized by Chinese Medical Association,Chinese Society of Gastroenterology,H.pylori and Peptic Ulcer Study Group was held at Hangzhou,Zhejiang Province on December 15-16,2016.More than 80 members of the study group and experts in the field of H.pylori research and gastroenterology attended the meeting.Consensus preparatory group was established to draft the related statements.The quality of evidence and strength of recommendation were evaluated by GRADE system.The Delphi method using an anonymous electronic system was adopted to develop the consensus of relevant statements.Expert advices about the established statements were firstly consulted through the emails.After two rounds of consultation,the preliminary statements of consensus were discussed and modified in the conference item by item.A total of 21 core members voted for the final version,which contained a total of 48 statements and was divided into 6 parts,including indications for H.pylori eradication,diagnosis,treatment,H.pylori and gastric cancer,H.pylori infection in special populations,H.pylori and gastrointestinal microbiota.

Invasive fungal disease(IFD) is a common yet highly lethal complication in patients with hematological malignancies receiving chemotherapy or stem cell transplantation, as well as immune suppressive conditions including aplastic anemia and other malignancies.According to the diagnostic criteria, patients are defined as proven, probable, possible and undefined IFD based on the evidence provided by histopathologic/cytologic, culture, radiographic and biomarker examinations.For the management of IFD, the major treatment strategies consist of prophylaxis, empirical, diagnostic-driven and target therapy.The Chinese Invasive Fungal Infection Working Group has developed the Chinese consensus for the diagnosis and treatment of invasive fungal disease based on international guidelines and local experience.Recently, the working group revises the consensus by update international guidelines and clinical studies in China.

Precise evaluation of the structure and function of collateral circulation is vital for individualized management in stroke.By summarizing the most up-to-date evidence, the new concepts and clinical significance of cerebral collaterals are highlighted in the guideline, and clinical recommendations for the evaluation and management strategies of cerebral collateral circulation are provided.

Endoscopic retrograde cholangiopancreatography (ERCP) was reported for the first time in 1970s. Since then, ERCP has become an indispensable procedure in the diagnosis and management of a variety of biliary and pancreatic disorders. The main purpose of Chinese ERCP guideline is to precisely define the clinical indications of ERCP. In this guideline, all recommendations were based on reviewed studies and graded on the strength of supporting evidence by GRADE criteria. ERCP is particularly appropriate in the management of patients with biliary obstruction caused by choledocholithiasis and other benign or malignant disorders. Meanwhile, a variety of benign or pancreatic disorders can be diagnosed and treated with ERCP. Plastic or metal biliary and pancreatic stents are primarily used to establish patency of an obstructed bile or pancreatic duct. Last but not least, it is very important to fully understand the potential complications of ERCP. Early recognition and immediate management of adverse events are critical to reduce procedure related morbidity and mortality.

Multiple myeloma ( MM) is a clonal plasma cell malignancy , mainly in elderly people and still incurable at present .In the ear of novel agents and sensitive laboratory exams , the diagnosis and treatment of MM have been significantly improved .Chinese MM guidelines for the diagnosis and treatment were updated every two years according to the progression of international and domestic research and clinical studies.In this version, we updated the response criteria and new combination regimens in newly diagnosed patients.Monoclonal gammopathy of renal significance ( MGRS) was added as a new part of differential diagnosis, meanwhile, relapsed/refractory MM patients should be treated as long as possible .
